Observation

During the complaint assessment it was found that an incident report dated November 2020 had been written documenting multiples bruises on the child’s arms that appeared to be fingerprints. The incident report was signed by a teacher and states that the supervisor notified was the center director. During the complaint assessment a teacher reported to the Licensor that the center director told the teacher that she called and made a CPS intake report regarding the bruises. It was also reported by a teacher that concerns over the child were brought to the director’s attention in July 2020 and the director reported to the teacher that she made an intake report to CPS. In an email to the Licensor, the center director stated that she called and made a CPS intake three times, however, there is no record of intake reports being made. Dispute Description: Staff states that all call to CPS were completed in a timely manner, and contest the statement that CPS was not notified of our concerns.
